Prepare for your visit

We look forward to seeing you soon at one of our practice locations! We put together a handy list of preparations so we can work together to ensure you have the best experience possible.

Before your visit

If you are a new patient, complete and submit the forms we text or email you. Please submit these digitally prior to your visit. If you are unable to submit them online, please print and complete them by hand and bring them to your visit.

Please plan to arrive 30 minutes early to find parking and complete any additional paperwork needed. 

If you are visiting for an ultrasound appointment, please review our Ultrasound Visitor Policy.

Bring these items with you to your appointment

  • Your photo identification.
  • Your up-to-date insurance information.
  • Method of payment to cover your co-pay or deductible.
  • Any forms you were unable to submit online.
  • If you did not submit your medications through our secure Patient Portal, please bring a list of all medications you are currently taking, including how many times per day and the dose strength of your medicine. You may find it easier to bring in your medication bottles or packages for the doctor to see.
  • Your list of questions and concerns for your doctor.

Finding your correct facility

We have three locations in Tallahassee and one in Carrabelle. Please visit our Locations and Directions page for assistance in finding your facility. Remember to double check your appointment confirmation for the location, as several of our providers see patients at multiple locations.

Once you arrive

We work hard to see patients close to their scheduled appointment times. There are a few factors that may cause delays, such as: high patient volumes, emergencies, unexpected issues with patients before you that required extra attention, etc.  We appreciate your patience if there is a brief delay. We encourage you to bring your phone, laptop or reading materials to our waiting rooms in case of a wait.

To make your time a more comfortable, we offer complimentary Wi-Fi in our waiting rooms. Look for the network titled: NFLWC Guest.

We encourage face coverings to protect against airborne illnesses; however, wearing them is a personal choice for patients, visitors, and staff within our facilities.
