Insurance
North Florida Women's Care accepts payment from a number of medical insurance plans. A list of plans we honor is available here. If we are a participating provider on your plan, we will accept your valid insurance card as payment for the services provided to you. If a co-payment or a deductible applies, you will be required to pay this at the time of service.
We will also file a claim on your behalf directly to your insurance company. Since insurance benefits (including co-payment and deductible amounts), billing addresses and eligibility change frequently for our patients, we rely on the information printed on your insurance card to file your claim properly.
Consequently, you must show your card at every visit. If you forget your insurance card, you will be expected to pay for your visit that day.
Patients who have come to North Florida Women's Care for years or others who return for frequent visits must present their insurance card at every visit.
Unfortunately, this is the only sure method for us to verify your insurance information.
Self-pay
If you do not have medical insurance, forgot your insurance card or we are not a participating provider on your plan, you will be considered a Self-pay patient. Self-pay patients must pay for the medical services they receive from us at the time of the visit. We accept cash, personal check, Visa, MasterCard, American Express or Discover.

- Vocational Rehab - First appointment made by the counselor. MUST obtain voucher from carrier.
We are happy to provide care to any patient with insurance coverage other than the plans listed above. We will file a courtesy claim to the insurance carrier however payment in full is required at time of service. We will be considered out-of-network for these visits and payment will typically be sent to the patient's home for the amount the insurance company is responsible to pay.
We are not participating providers for GYN Medicaid or any other type of Medicaid including (Healthease, Universal, Medically Needy).
